6/1/2023 EnchantaVaughn Approved Behavior: At the beginning of the session, client expressed how he feels overwhelmed at times. Client wants to be able to balance everything and still be a kid. Client is afraid that he will be held liable for anything that goes wrong since heâs the oldest sibling. Client expresses that he misses having the attention to himself and wish he had his own room to do more older stuff. Behavior: Client was apologetic for the way that he responded to what his mother was saying about his past behaviors and stated he wanted to fix any problem he had. Client shared that he will work on building a better relationship with his mother, if his mom would let him do more for himself even if that means it takes him longer to do so. Mother shared that she allows client to do more than he should. Intervention: QMHP commended client on utilizing creative strategies to increase his understanding of feelings and emotions. QMHP processed client feelings and offered choices of what to do when faced with conflict or challenged in the home. QMHP will assist with reducing anger-provoking situations through self-talk and role play scenarios in order to prevent escalation and dealing with anger. Intervention: QMHP practiced calming techniques with client that can be used in moments of high stress and anxiety. QMHP educated client about using good manners at home and offering alternative ways to model appropriate language that client should use to ask for things or to get someoneâs attention. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support. Response: Client communicated that he felt safe and comfortable talking to the in-home counselor. Both client and guardian provided feedback, actively participated, and engaged with QMHP. Client was slightly hyperactive and required one on one attention to reduce symptoms. Client was provided with redirection for displaying emotional distress to minor irritants. Response: Client was receptive and open to the interventions, as he was attentive and focused on the information and activities that were being processed without any difficulties. Client demonstrates problem solving by staying calm and using his senses. Client displays good social behavior, as observed and supported by QMHP. Client is reported building healthy relationships in the home.
